Dear Sir/Madam,

ISSUANCE AND RENEWAL OF MALAYSIAN PASSPORT
BORANG IM.42 - FOR NEW PASSPORT / BORANG ‘C’ - RENEWAL OF PASSPORT

- Current Malaysian Passport
- 2 Copies of Malaysian Identity Card certified by a Notary Public (Front & Back)
- 2 Copies of Birth Certificate certified by a Notary Public
- 2 Copies of Citizenship Certificate certified by a Notary Public (if applicable)
- 2 Copies of Marriage Certificate certified by a Notary Public (if applicable)
- 2 Copies of Permanent Resident Card certified by a Notary Public
- 2 Copies of letter from your employer/school 
  confirming your status (photocopy accepted)
- 2 Copies proof of visa status, if your visa is attached in 
  your old passport, kindly provide certified copy.
- The following fee via US Postal Money Order (NO PERSONAL CHECK) 
  made payable to Embassy of Malaysia:
   a) US$80.00 (for 32 pages) OR US$160.00 (for 64 pages)
   b) US$40.00 (for 32 pages) OR US$80.00 (for 64 pages for children and 
      students below 21 years of age.
- Four (4) passport-sized - Borang IM.42/ Two (2) passport-sized 
- Borang C ; front view  full face photographs taken within 2 weeks
- For return postage, please provide $14.40 (stamps).

IMPORTANT:  Please be sure to place your right thumb print as indicated 
on the application form IM.42 for new passport only.

NOTE:
1.	Holder of Malaysian passport is reminded to keep the passport 
valid at all times for personal convenience.  It is advisable that application 
for a new passport be submitted six (6) months before its expiry date.

2.	The Embassy of Malaysia, Consular Division will again process 
and issue new passport (old version) in our office in Washington, D.C.  
Therefore, issuing a new passport (with microchip) will be forwarded 
to the Immigration Headquarters in Kuala Lumpur, Malaysia for processing.  

3.	To ensure a prompt processing please make sure that all 
supporting documents are certified by a Notary Public, failure 
to do so will only delay the process of your application.

4.	For every case of lost Malaysian passport it must go through 
a through process of investigation by the Immigration Headquarters in 
Kuala Lumpur before any consideration can be made for its replacement.  
As an important document, the passport should therefore be in the safe 
custody of the holder at all times.

SPECIAL ANNOUNCEMENT:

Please be informed that the process of issuance of a new or renewal of 
passport would depend on the following manner:
		1.	Validity of Visa OR Status in USA 
		2.	Invalid Passport for a long period of time.	 

All application will be forwarded to Immigration Headquarters in Kuala Lumpur, 
Malaysia for approval.  The estimated time of processing may take up to 3 months or more.
