All PETRONAS Scholars.


PLEASE BE REMINDED that upon reporting, all graduates <b><u><i>must submit</i></u></b> the following documents:

   1. Lapor Diri* form which can be downloaded from <A style="TEXT-DECORATION: none" href="http://www.petronaswdc.com/forms/Borang Lapor Diri - new 221209.doc" target=content><u>here</u></A> or ESU Portal; and
   2. A certified true copy (CTC) of the Final Exam Results or Exam's Transcript
   3. 1 passport-sized photo
   4. 1 copy of IC or Passport (CTC)
   5. 1 copy of SPM result (CTC)
   6. 1 copy of Scroll (CTC) if available
   7. 1 copy of CV for recruitment purposes


PLEASE BE REMINDED ALSO that failure to report ("lapor diri") upon completion of sponsoship
shall constitute a breach of the terms and condidtions of the Agreement and any modification
made thereunder, and PETRONAS shall be absolutely entitled to exercise its rights and remedies
without any further reference thereto.


Do take note that I have posted an announcement on the new ways or methods for
you to report to ESU upon completion of your studies on 11 May 2009.

* this is the latest Lapor Diri form. If you have printed the earlier version of the form, 
you are required to download and fill up the new form as the old form will no longer valid.
